What causes auto-PEEP in patients?

Auto-PEEP, or intrinsicPEEP, occurs when there is insufficient time for the lungs to fully exhale before the next breath is initiated. This can lead to the trapping of air in the lungs, causing the end-expiratory lung volume to be elevated above the normal resting level. When patients have reduced expiratory time, such as in conditions of increased airway resistance or prolonged inspiratory time due to ventilator settings or patient effort, they may not be able to fully exhale, leading to the gradual accumulation of air within the alveoli.

This accumulation prevents the full release of pressure from the lungs, resulting in auto-PEEP. By ensuring that patients have enough expiratory time, healthcare providers can help prevent this condition, as it is vital for effective gas exchange and preventing over-distension of the lungs.
